POSITION REQUIREMENTS

POSITION OVERVIEW:

The Technical Writer delivers high quality, clearly written application documentation for CHW Medical Foundation.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.

• Maintains library of all CAS policies and procedures.
• Creates, edits, and maintains procedural documentation including but not limited to department policies and procedures application guides. The materials are then produced in a variety of media including print, PDF, online help, video, etc.
• Writes, edits, produces, and maintains on-line (internet/web-based) documentation support quality assurance efforts, assemble documentation for audits and ensure that documentation is compliant with federal regulations.
• Adheres to prescribed documentation formats, guidelines and standards.
• Designs and develops documentation style standards and templates.
• Researches and evaluates new documentation tools and methods; document and improve documentation processes.
• Monitors compliance to standards by reviewing and editing business and technical communications deliverables.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Required:
• 5+ years of information systems experience.
• 3+ years documenting a commercial software product, preferably in the healthcare IT industry.
• Ability to work in a complex and matrixed team environment; experience working in a team-oriented, collaborative environment.
• Excellent verbal and written communication skills.
• Skills in Microsoft Project, Word, Excel, PowerPoint, and Visio.
• Ability to handle multiple tasks, set priorities, schedule, and meet deadlines.
• Possesses the ability to provide sound and reliable judgment, take initiative, and accomplish task with minimal supervision in complex situations.

Preferred:
• Associate or Bachelor’s Degree in Journalism, English, Technical Communications, Computer Science or related field.
• Working knowledge of IDX
• Working knowledge of Allscrips
• Highly self-motivated, self-directed, and attentive to detail.

Three years of medical office experience. Excellent customer service communication and computer skills.Experience with HMO/PPO Insurances, 3 years processing referals. OB/GYN experience preferred. Excecellent follow up skills, Must communicate well with outside facilities and the UM department. Must check status daily on all pending authorizations, must input all new requests within 24 hours.

Process patient requests for referrals and authorizations both internal and external; encounter form processing and batching; scheduling in/out patient medical procedures; greet arriving patients, manage patient wait times; assist patients with necessary completion of paperwork; provide informational services registration of all patients; process incoming/outgoing calls/schedule appointments; other duties as assigned.

POSITION REQUIREMENTS

Ophthalmic medical personnel perform assigned duties as assistants of the ophthalmologists or optometrists. Obtain preliminary information by taking a patient history and perform testing to be used by the ophthalmologist or OD.

Ensure appropriate data is collected from patients physically or telephonically in order for MD to determine course of action.

Perform visual fields and ASCANS as nurse visits.

Ensure receptionists schedule emergency (same day) patients appropriately. (In enough time for dilation procedure if necessary).

Ensure appropriate data is transmitted to OOG specialist.

Perform proficiency testing for MA’s and COA’s.

Assist in upgrading knowledge base to MA’s and COA’s for ophthalmologic skills. (COT only)

Take patient histories: ascertain complete medical and ophthalmic and ocular histories, systemic illness (past and present), family histories, medications, and allergies.

Performs basic ophthalmic and ocular skill and lensometry including measuring/recording acuities color, Amslergrij, and Schirmer testing. Evaluation of pupil and estimation of anterior chamber depth. Ability to collect appropriate data for physician/OD evaluation and assessment.

Prepare UR reviews and patient scheduling for OOG specialist. Ascertain information necessary for referral.

Performs automated visual Field test, clinical optics and lensometry/retinoscopy. Performs ocular motility by evaluation assessment methods (COT only).

Schedules and handles all preoperative, preparation with ocular surgical patients including surgical instrumentation set up and scrubbing for in office surgical setting. Ability to select appropriate surgical instruments, equipment prepared in procedures room.

Performance of CPR by certification.

Acquire necessary CE units for recertification.

Instruct patients for contact lens fittings (COT only)
Ability to take only pertinent patient history for ophthalmologist to determine/assess patient. This must be performed in person or telephonically on a daily basis.

Ability to document ophthalmologist findings accurately and legible.

Ability to communicate with patient, family members, and referring staff to obtain necessary information during initial contact and ensure immediate referrals are seen by ophthalmologist immediately.

Qualifications:
- Completion of an accredited medical assisting program
- Current CPR
- Good customer service

Removal and/or application of dressings for assessment by licensed provider, cleaning or irrigating would after provider’s assessment and instructions, ear washes etc. Some data entry for obtaining treatment and DME authorizations. Scheduling patient appointments and surgeries. Preparing patient charts. Other duties as assigned. Travel between multiple site and departments required, position will include short and long term coverage in primary care, medical specialty and surgical departments. Flexibility with work hours. POSITION REQUIREMENTS

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.
? Oversee all mergers and acquisitions including change management initiatives.
? Conduct initial due diligence on technology and clinic/practice operations.
? Manages the implementation of clinical applications and software to insure desired clinical and business goals and objectives are met. Leads teams through the change process.
? Builds relationships with peers, and staff and utilizes proficient communication skills to communicate effectively with all levels of the organization..
? Ability to provide initial diagnoses of equipment problems; fixes or seeks out the appropriate resource to correct the problem. Consistently informs appropriate person of problems encountered with repairs or calibration of equipment.
? Utilize continuous process improvement principles to assure effective CHWMF operations implementation.
? Serve as primary contact for 24x7 support to Implementation Team.
? Oversee Best Practice Implementation process and Quality Assurance.
? Provide leadership and oversight for Implementation Team to ensure that objectives are met and work is distributed and accomplished ina timely fashion, meeting the needs of patients.
? Collaborates with CHWMF contributors, including physicians, professionals using both manual and automated tools to ensure appropriate, effective usage and understanding to ensure maximum efficiency and clinical outcomes.
? Define, develop and implement new processes and policies throughout CHWMF clinics.
? Sets strategic direction of projects. Develops project plans which identify key issues, problems, approaches, performance metrics and resources required.
? Accountable for the development of project documentation for senior executives and other key clients to facilitate sharing of project outcomes and best practices.
? Maintains confidentiality of all patient and hospital-related information, communications and documents.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Required:
• Bachelor’s degree required preferably in Health Informatics or Computer Science
• Minimum 9 years experience in information systems applying analytical methodologies, with specific experience utilizing an Electronic Medical Record system and/or Information interfaces.
• Minimum 5 years experience in a leadership role.
• Comprehensive working knowledge of IDX
• Comprehensive working knowledge of Allscrips
• HL7, .NET, Dragon knowledge required.
• Position is mobile by nature and requires 90% travel
• Experience working in an outpatient physician practice / ambulatory setting with direct responsibility for patient care activities in a leadership capacity.
• Ambulatory care/outpatient environment
• Demonstrated skill in and understanding of data standards and interfaces.
• Comprehensive working knowledge of IDX
• Comprehensive working knowledge of Allscrips
• Understanding of HIPPA, security and compliance regulations.
• Strong problem solving skills, ability to manage project tasks and timelines, and analytical ability.

POSITION OVERVIEW:
This position is a key member of the CHWMF Hospital Medicine Department leadership team and is responsible for developing and coordinating various initiatives that focus on identification and management of improving quality of service/care and efficient operations while maintaining cost effectiveness. The Hospital Medicine Department oversees physician staffing coordination 24x7. This position serves as a key contact for review of clinical data for the hospital medicine program. Through assessment of patient data, chart reviews, care management reports, identifies significant trends and develops reports to communicate analysis to leadership, Administrative Director and/or appropriate physicians to impact physician practice.
This position works independently using high level critical thinking and also in conjunction with the Director of Hospital Medicine Dept. Operations and Medical Director, Hospital Medicine in the development, coordination, implementation and management of the department operations. The position requires coordination with hospital staff, physician leadership, and business partners, often under tight deadlines and with multiple projects to balance and to successfully complete.


ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.
Daily Operations:
• Facilitate and communicate deliverables as designated by director.
• Interfacing with TC team on hospital initiative that involves Hospitalists
• Develop timelines and track status of initiatives including length of stay, bed days, and quality performance.
• Track expenses against the budgets for the Hospital Medicine program.
• Employee supervision of support staff (FTE) to ensure daily operations (including 24/7 MD Staffing)
• Utilize Cerner EMR, Allscripts EMR to review physician performance to specified indicators
• Responsible for “on boarding” new physicians including facilitating the credentialing, orientation and proctoring process.
• Serve as resource for facility MMG physicians, hospital privileges.
• Responsible for overseeing process of charge capturing, compliance.
• Oversees the development and documentation of Hospital Medicine Department policies and procedures.
• Partner with MMG to meet agreed upon deliverables.

Quality Activities:
• Working in concert with the hospital quality assurance departments, ensures that patient care meets JCAHO standards and hospital parameters. Implements action plans when variances occur.
• Oversees the quality improvement program in conjunction with various contracted entities such as Mercy Medical Group, hospitals and plan plans. Programs include, but not limited to readmission review, patient satisfaction, PCP satisfaction.
• Develop, implement and monitor policy and procedure improvements as a result of ongoing review of oversight programs.
• Contribute to the development of quality improvement projects, initiatives
• Work in collaboration with marketing department on marketing efforts related to quality improvement initiatives - patient satisfaction
• Work in collaboration with the IT department on IT efforts in support of HOSPITAL MEDICINE PROGRAM initiatives – interfacing flow of information between Hospital Clinic Information Systems
• Oversee the activities of HOSPITAL MEDICINE PROGRAM QI-related committees and task forces to achieve QI strategies and priorities
• Increase Patient Satisfaction through patient education programs, marketing and quality initiatives

POSITION REQUIREMENTS

This position oversees and performs daily office operations seven days per week for the hospital medicine program. Due to the intricacy of the job, this individual must be extremely organized and possess the skills require to function independently, utilizing critical thinking in problem resolution with little day-to-day supervision. The ability to interpret and apply specialized technical or program knowledge while performing the duties is required. This position regularly performs a variety of complex and multifaceted duties which include:

1. Coordination and appropriate assignment of ED admissions with hospitalist for efficient throughput of patients.
2. Accurately and consistently provides oversight for and prepares hospitalist census reports through daily tracking and coordination of patient flow within the Hospitalist Program including patient master service list.
3. Oversees the completion and reconciliation of the E-billing system (Allscripts) and works in conjunction with the Business Office and or Medical Records to assure timely completion.
4. Prepares and develops logs, records, schedules, files and manuals; prepares and/or types a variety of reports, charts, graphs and other materials, utilizing both manual and computer generated data with accuracy 95% of the time.
5. Facilitates patient/physician identification and tracking as well as education when needed as measured through feedback from physicians and managers. Superuser for Allscripts, IDX, PowerChart. Extracts MS4 data and provide corrected data to appropriate parties, as needed. Provides support and educates physicians. Prepares condolence cards as needed.
6. Acts as liaison between the Hospitalist Medicine Program, Hospital, Medical Groups and Health Plans, and ensures timely notification submission to PCP offices, while maintaining accurate patient data.
7. Maintains office organization, including ordering supplies and keeping files current including reconciliation of lab coat inventory and billing.
8. Trains, assigns and/or reviews work, and provides direction to other program support staff members when needed.
9. Acts as liaison between the hospital, hospitalist and funeral home for accurate and timely death certificates filings in compliance with state regulations.
10. Manages prescription clarification coordination between the hospitalist and pharmacies when needed.
10.11. Appropriately triages internal/external customer calls at point of initial contact.
11.12. Performs other duties as assigned per manager and/or Hospital Medicine administrative team.

QUALIFICATIONS:
The minimum requirements for this position include an Associate of Arts degree or its equivalent, at least two years experience performing related duties with at least one in the acute hospital and one year in managed care. An equivalent combination of education and experience which provides the following knowledge, skills and abilities may be considered.

Knowledge of:
• Healthcare insurance, managed care, managed care contracting
• Regulatory agency requirements for above
• Business mathematics and statistical math skills
• Business office procedures, correspondence and filing systems
• Personal computers, word processing and database applications, including Word, Excel, Cerner, MS4, Flowchart, IBAX, HSSI, HBI, Allscripts.

POSITION REQUIREMENTS

Position Overview
Responsible for the strategic development, implementation and evaluation of compliance management programs within CHW Medical Foundation in alignment with CHW policies, procedures and practices. Acts as the internal expert on all compliance issues to ensure adherence to all corporate standards and policies while consulting with CHW Medical Foundation executive leadership on issues and opportunities to achieve sound business practices in the area of compliance.

Duties include, but are not limited to:

• Supports the CHW compliance principles by maintaining the privacy and confidentiality of information, protecting the assets of the organization, acting in accordance with CHW’s Mission and Core Values, providing education on areas of non-compliance, and adhering to applicable federal, state and local laws and regulations, accreditation and licensure requirements.
• Ensures that CHW Medical Foundation has systems in place to achieve compliance through the development of compliance management systems including training, policies, procedures, monitoring and auditing across all areas of the organization.
• Assists members of the leadership team in adopting a culture of compliance in their daily operations.
• Identifies areas of risk for compliance deficiencies and develops, implements, tracks and reports on work plans for correction and adherence to policies and standards.
• Participates in the development and implementation of audits for internal, regulatory and health plan compliance.
• Intervention and process development based on audit deficiencies which may include identification of needed policy development and education, procedures and regulations interpretation and application. This individual will provide leadership with the regulations and requirements. Applicable leadership will develop and implement policies within the facilities. The compliance department will then audit for compliance with the internal policies and processes.
• Works with the CHWMF Education Department on compliance curriculum to ensure all corporate requirements are being met.
• Provides/facilitates mandatory annual training of all physicians and coding staff.
• Provide Corporate leadership with information to be included in the monthly disclosure reports and board reports as prescribed by CHW policies and practices.
• Acts as the CHW Medical Foundation expert on regulatory compliance – remaining up-to-date on new regulations and work with Executive Leadership to implementation strategies as required within CHWMF.
• Communicate and oversee compliance aspects of the President’s Scorecard.
• Responsible for communicating all CHW requirements regarding background and compliance checks. (These checks are the responsibility of HR, with compliance committee oversight. The Credentialing Department does the exclusion lists.)
• Works with leadership of the various functional areas to ensure all relevant policies and procedures are reviewed regularly and maintained including training updates with affected staff. May have input into policies as required. Individual functional leadership is responsible for development and writing of the policies.
• Chairs the compliance committee meetings quarterly minimum with agenda and minutes.
• Manages the coding audit process to ensure all physicians are audited annually including the development of corrective and educational action plans specific to opportunities identified during the audits.
• Interact with Corporate Compliance on issues related to adherence to corporate standards and identified deficiencies, working with executive leadership to create and implement new processes and corrective actions.
• Maintains regular reporting to Corporate Compliance to ensure corporate standards are met and mentoring occurs.
• Responds to various enquiries and work requests associated with compliance issues.
• Acts as liaison between CHW Corporate Compliance and CHWMF Executive Team and Mercy Medical Group Medical Director.

QUALIFICATIONS

• Bachelor’s degree in Business Administration, Health Care Administration, Nursing, Public Health Administration, Law, or other related field or equivalent experience.
• Certified Coding Background strongly preferred.
• Minimum of 7 years’ experience in a combination of increasingly responsible roles in compliance, and/or coding and/or auditing.
• Ability to read and interpret regulations and implement appropriate responses for CHWMF.
• Ability to provide leadership to, and influence members of CHW Medical Foundation’s leadership up to and including its executive leadership.
• Demonstrated ability to lead change.
• Excellent judgment and decision-making skills.

The Clinical Performance Specialist is part of the Implementation and Best Practices Implementation Team and serves as a resource to physicians and operations in identifying training and resource needs. ? Utilize simulation training.
? Conduct skill set assessment for MA and Supervisor competency for training curriculum development and delivery.
? Conduct systems trainings at New Provider Orientation.
? Participate in the development and execution of strategic projects including the development of project plans, key issues, problems, approaches, performance metrics and resources required.

POSITION REQUIREMENTS

This position performs daily audits of non-office encounter forms and specialty office encounter forms, and chart documentation to identify educational opportunities and areas of potential risk with regulatory agencies. This position is responsible for identifying areas of non-compliance in both the areas of coding practices and quality medical record documentation. This position also has the responsibility to resolve claim edits resulting from the Transaction Editing System (TES). Any reported issues resulting from the encounter audit or clearing edits, this position will report back to the provider recommendations on improving their coding behaviors. This position will be responsible for adhering to established processes and procedures throughout the organization that maintain billing integrity and timelines. This position will be responsible for maintaining current knowledge of the various regulatory agency guidelines and requirements and will work with staff and providers to ensure that all regulations are met, and that claims are subsequently submitted to payers with accurate and compliant data.

Education: Certified Professional Coder

Certification/Licensure: Certified Professional Coder (CPC) preferred
Experience: 5 (5) years coding experience. Experience with physician practice coding and billing reviews. Knowledge of Medicare, MediCal and third-party payer coding and billing regulations.

Skills/Knowledge: Excellent written and verbal communication skills, including the ability to present ideas and concepts effectively to physicians and coding staff. Skill in examining and evaluating data with references to standards and requirements. Excellent analytical and problem solving abilities as it relates to medical coding issues.

POSITION OVERVIEW:

Under the supervision of the A/R Supervisor, the A/R Billing Rep II is responsible for providing timely and efficient follow-up with all payer types for all unpaid claims, consistent with the Mission and Philosophy of CHW Medical Foundation.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.

? Contact payers to verify status of unpaid claims.
? Respond to complex inquiries and correspondence related to unpaid claims.
? Follow-up with insurance carriers and CHWMF staff for information essential for proper billing and collections.
? Analyze and resolve underpaid claims.
? Work with Refund Analyst to resolve overpaid claims.
? Validate insurance information.
? Resolve claim edits.
? Special projects from management team.
? All other duties as assigned.

Under the supervision of the A/R Supervisor, the A/R Billing Rep II is responsible for providing timely and efficient follow-up with all payer types for all unpaid claims, consistent with the Mission and Philosophy of CHW Medical Foundation.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.

? Contact payers to verify status of unpaid claims.
? Respond to complex inquiries and correspondence related to unpaid claims.
? Follow-up with insurance carriers and CHWMF staff for information essential for proper billing and collections.
? Analyze and resolve underpaid claims.
? Work with Refund Analyst to resolve overpaid claims.
? Validate insurance information.
? Resolve claim edits.
? Special projects from management team.
? All other duties as assigned.

QUALIFICATIONS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. ? High School Diploma or GED required.
? Two years experience in professional medical billing office.
? Knowledge of physician billing regulations.
? Understanding of professional claims and billing procedures.
? Knowledge of Federal and State regulations applicable to Government and Insurance Collections.
? Excellent written and verbal communication skills.
? Working knowledge of computers and demonstrated proficiency in using e-mail systems, Internet and MS office software applications with emphasis in Word and Excel.
